Absence Excuse Note

Complete this form to report your child's absence.

  • Absences are only ‘excused’ if they are one of the types on the school’s list of acceptable excuses. A parent or guardian’s permission is not enough to make an absence ‘excused.’ Absences will only be ‘excused’  for the following reasons:
    • Student Illness/sickness (Doctor's note is need if abscent for 5 days or more)
    • Medical/Dental Appointment
    • Student Judicial Proceeding  (please provide document from court verifying presence)
    • Religous Holiday
    • Death in the Family
    • Other Circumstances Approved by DCPS
